Diabetes Self Management Education Group (DSME)

What is the Diabetes Self-Management Education (DSME) Group?

The DSME is a group clinic to learn how to take charge of your diabetes. You are welcome to bring a family member or support person with you. There is a lot to learn; plan to stay for 2 hours. In addition to our clinic patients we take referrals from outside providers as well. For this program you do not need to be uninsured. You can keep your primary care provider and come to us for Education only. If you do not have insurance and have diabetes we will also help with your medical needs if you qualify with our income criteria Diabetes Self-Management Education (DSME) is the cornerstone of care for all individuals with diabetes who want to achieve successful health outcomes and avoid complications.

The program consists of four sessions over the course of 3 months for total of 6 hours. Each week focuses on a specific topic.Sessions are facilitated from a highly detailed manual from DHEC by two trained instructors. Classes are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives. Each participant in the workshop receives a copy of the companion book and session notes.

Diabetes Prevention Program

What is the National Diabetes Prevention Program?

The National Diabetes Prevention Program (DPP) is an evidence-based lifestyle change program which has been demonstrated to delay or prevent the development of type 2 diabetes among people at high risk. The program consists of trained lifestyle coach who facilitates a small group of adults to discuss behavior changes that can improve the health of participants. The program consists of 25 one-hour sessions delivered over the course of a year.

Program Goals
  • Lose 5–7% of your body weight
  • Gradually increase your physical activity to 150 minutes per week.

Cooking Matters Classes: Held by Low Country Food Bank

Cooking Matters signature courses are six week culinary-based nutrition programs.These courses empower our participants with the knowledge to select and cook the most nutritious meals possible for their families. Courses are offered for children, teens, adults and families.
